docs: add comprehensive logical audit report
Adds a full cross-document audit covering:
- Data Models (broken refs, ghost states, missing constraints)
- API Reference (unauthenticated endpoints, field mismatches, missing pagination)
- Architecture (fictitious deps, statelessness claims vs reality)
